What should I expect on my first visit?

After our discovery call, we will send you our Patient Intake Forms and directions to our facility. The initial evaluation will be 60-75 minutes where we will discuss your injury, current activity level, and future goals.

From there, your performance PT will go through a full body evaluation which will include range of motion testing, strength, functional movement patterns, and force/power testing if applicable. This will help us initiate your plan of care moving forward.

Do you take my insurance?

Lakás PT & Performance is an Out of Network Provider. This means that your rehab journey is individualized by goals YOU set out, and not predicated on insurance companies' guidelines.

You may be reimbursed a certain % of your PT services if you have a PPO. We recommend calling your insurance company to verify your Out-of-Network Benefits and can provide you a script to help you navigate this. We do provide superbills for those who have out of network benefits.

We also offer payment plans on services and also take HSA and/or FSA.

How is physical therapy at Lakás PT different from other clinics?

We look at the site of the symptoms as well as the whole body because we believe that everything is connected. Our approach is holistic and all-inclusive because we know that optimizing movement in order to relieve you of pain and dysfunction requires an approach that takes everything into consideration. One session will always build on the next and we make sure that we are addressing your specific goals.
